supporting quality nursing services to enhance
the health and well-being of individuals,
families, and communities.
Nurse-Family Partnership Program
The Nurse-Family Partnership Program begins early in
the pregnancy and continues until the child’s second
birthday.
Program Eligibility:
First-time pregnant women (less than 28 weeks
pregnant)
Maternal Child Health Program
The Maternal Child Health Program may begin during
any stage of pregnancy or early parenting, the length
of service varies.
Program Eligibility:
Pregnant, Postpartum or Early Parenting Families
